President
Lisa Schaffer
Lisa brings over 30 years of diverse nursing experience and a deep commitment to patient advocacy. Her clinical background includes roles in medical-surgical nursing, intermediate care nursing, burn unit nursing, private duty nursing, charge nurse responsibilities, and volunteer service as a camp nurse. Following her time in bedside care, Lisa pursued further education and earned her Bachelor of Science in Nursing (BSN). She holds professional certifications in Infection Control (CIC), Certified Rehabilitation Registered Nurse (CRRN) and Certified Case Manager (CCM) and has actively contributed to numerous interdisciplinary teams focused on Performance Improvement, Quality Assurance, Education, Survey Readiness, Hazardous Materials, and Emergency Preparedness. Lisa played a key role in the development of Nevada’s original Inter-Facility Infection Control Transfer Form, an experience that underscored the critical importance of collaboration beyond individual healthcare facilities. In 2025, Lisa received two awards related to her role as a case manager, Excellence in Nursing 2025 (Case Management Category) presented by The Arc of Nevada and 2025 Excellence in Nursing – Women & Leadership (Case Management Category) presented by Las Vegas Healthcare and Vegas Heals. Her professional goals include fostering networking and collaboration among Case Managers, Social Workers and the many community resources available in Southern Nevada.

Vice President
Kerry Halcomb
Kerry Halcomb, Physical Therapist, is a passionate healthcare leader with over 25 years of experience in home health, hospice, private duty, and outpatient physical therapy clinics. Kerry is known for identifying and executing strategic growth opportunities, enhancing operations. He has been successful in growing a home health agency in Arizona for 14 years until merging with another company. Kerry is now the Regional Director of Operations of AdvisaCare Home Health and Hospice for Arizona and Nevada. In his spare time, he serves on many healthcare related boards including (AAHC) Arizona Association Home Care, (CMSA Arizona) Case Management Society of America Arizona, (AALHA) Arizona Assisted Living Homes Association, and (RACC) Romanian-American Chamber of Commerce and (CMSA Las Vegas) Case Management Society of America Las Vegas.

Treasurer
Mohammad Ali Pourteymaur
Mo Pourteymaur, PharmD, has been a pharmacist in the Southern Nevada community for almost 30 years. His experience ranges from hospital pharmacies to retail pharmacies, and his passion spans over 25 years in the home infusion pharmacy space. Mo began as a Staff Pharmacist at Total Infusion, now LUX Infusion, 14 years ago and is now the Pharmacy Manager of LUX Infusion, as well as one of the owners for almost seven years. He is an avid Denver sports fan and enjoys traveling back to Denver from time to time. Mo is committed to growing the membership of the Case Management Society of America Las Vegas and managing the finances.

Secretary
Denise Talamantes
Denise Talamantes is a trauma-informed holistic integrative practitioner serving Southern Nevada for over 15 years. She is the owner of Optima Wellness Center and is CISM-certified. She specializes in drug-free, evidence-based support for first responders, healthcare workers, and individuals affected by anxiety, PTSD, grief, addiction, insomnia, and chronic pain. Her integrative approach includes Reiki Therapy, Neurofeedback, Resiliency Life Coaching, and intensive wellness training to reduce compassion fatigue and support long-term well-being.
Denise also serves as Director of Business Development and Social Media Manager for Rhythm of Life Hospice and is a member of the Las Vegas Hospice Coalition advocating for patient rights and ethical hospice care. She provides end-of-life Reiki and integrative support to help patients find comfort and peace. In her free time, Denise enjoys hiking, traveling, cooking with her husband, spending time with family, and cheering on her grandson at baseball games and the Chicago Bears.

Sabrina Tenille Patrick
LORIAN HEALTH
Sabrina T. Patrick has been a healthcare business development professional in the Southern Nevada community for over 12 years. Her experience ranges from home health and hospice growth strategy to primary care and rehab, and her passion spans the full continuum of post-acute care and patient-centered collaboration. Sabrina currently serves as a Care Specialist Director at Lorian Health, partnering with physicians and care teams to expand access to innovative therapies and treatment. She is proud to serve on the Board of Directors for the Case Management Society of America Las Vegas and Sabrina is committed to strengthening interdisciplinary relationships and growing CMSA Las Vegas membership across the Las Vegas healthcare community. She enjoys spending time with her family and supporting meaningful community initiatives.
